Appointments- 
Patients are seen on an appointment basis, and we reserve time specifically with you and the doctor. We ask that you complete an online appointment request or call in advance so that we may reserve an appointment time for you.

If you are a new patient to our office, the link below contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing forms, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all. Emergency Care

For any life-threatening emergency, please call 911. For any other calls or to speak with the on-call physician, please call the office number and you will be redirected to the answering service for the on-call physician.

Telephone Calls
If you have any questions regarding your treatment, please feel free to contact us. Our office staff is qualified to answer most questions. If your doctor must be consulted, he will return your call at his earliest convenience.

Payment Options
For your convenience, we accept cash, personal checks, money orders and most major credit cards. Payment is expected at the time services are performed. When more extensive care is necessary, financial arrangements can be made with our office. We also offer CareCredit payment options for our cosmetic surgery patients.

Insurance
Medicare, United Healthcare, Secure Horizons, Anthem BC/BS, Aetna, Tricare, Cofinity, Rocky Mountain Health Plans, Kaiser Permanente, CIGNA and Medical Network . Please contact our office for more information regarding any other insurance carriers.
